Case Manager/COTA - FUSE Program

ReVive Center is seeking a full-time Certified Occupational Therapy Assistant/ Case Manager to join our FUSE team. This position is responsible for providing intensive, strengths-based case management services and occupational therapy interventions to families who are frequent users of the shelter system. This position helps families navigate housing access, stabilize routines and strengthen pathways to long-term housing.

ReVive Center is seeking a full-time Clinical Case Manager to join our Street Outreach program. The Case Manager is responsible for providing direct clinical case management services to individuals in our street outreach program including: intake and assessment, collaborative development of service plans, crisis intervention, individual counseling and therapeutic group activities as appropriate to support participants in meeting their goals.
